Season 2/episode 14 " A wolf in the fold".When Hengist dies Bones really said "he's dead Jim" contrary to many who decry the fact. I just watched/heard it myself. THere were many variants of the phrase uttered over the course of the show but really he's dead Jim until he isn't and gets beamed into space just before the end of the show and Scotty is off the hook for murder!
I just watched the entire TOS, and I recall Wolf In The Fold was one of our favs as a kid. I missed an opportunity to chuckle though, blowing by "He's dead Jim". I will say, there were lots of notification by the chief surgeon to the captain, of the untimely demise of a crewman or other unfortunate alien. I can't recall: were there any female casualties?
I still catch TOS episodes many nights, as they are on H&I TV 6 nights a week. They play one episode from each series from TOS to Enterprise, though that's past my bedtime. This was on recently:

I didn’t keep track of how many times Bones said “he’s dead, Jim”, but if there was a landing party that included a crew member never seen before and wearing a red shirt, you could be assured he wouldn’t be making it back to the ship. There's a couple more unlucky TOS ladies: Lieutenant Elizabeth Dehner, killed by Gary Mitchell, "Where No Man Has Gone Before". Lieutenant Arlene Galway, rapid aging, "The Deadly Years".
